Access Your Health Information Online
Please take a few minutes to register and you can have access to information and services provided in a secure and confidential manner. Please note that by clicking the link, you will be redirected to an external website. Once registered and logged on, you will be able to:
Exchange secure messages with our practice
Review and pay billing statements online

To Register
Click Portal Login above and you'll be redirected to the Athena Health patient portal.
Select New User Registration on the portal page to begin setting up your account.
Enter your First Name, Last Name, Date of Birth, and Home Phone Number exactly as they are on file with our practice.
Proceed as directed to complete registration and create your personal 5–13 digit PIN.
Existing Patients
To Log In
Enter your 5–13 numeric digit PIN — this is the number you created when registering for the portal.
Enter your eight-digit date of birth using MM/DD/YYYY format. Example: September 4, 1980 = 09/04/1980
Enter your 10-digit phone number including area code (the one on file with our office).
Click the Login button to access your account, review statements, and manage your care.
